To Whom It May Concern:
This letter will serve to confirm that __________________________________________grants
permission for cable company _______________________________________________ to
install and connect additional cable outlets at
__________________________________________________
__________________________________________________, Ohio contingent upon the following:
1. All required wiring to service the new cable outlets shall be ran through the attic and fished
into the appropriate wall section to the cable box.
2. No wiring shall be exposed on any interior wall or across any interior floor area in the
building.
3. No wiring shall be exposed on any exterior wall or across the roof of the building.
4. Any required trenching for wire drops shall be backfilled in a timely manner at the sole
expense of Cable Company _______________________________________________
5. All work to be performed shall be done in a professional manner in accordance with
accepted industry standards.
6. In the event any firewall is penetrated, any penetrations shall be sealed with fire code
caulking approved by Building Code.
7. Satellite Dish must be installed at rear of property, in a manner that dish is not seen from
street. Satellite Dish cannot
be attached to building in any way or any patio fence.
8. It is understood and agreed between the parties that

Apartments will incur no expense as a result of any installation
ordered by our Lessee.
9. Lessee must remove Satellite Dish and restore landscaping to original condition upon
vacating the premises. Lessee’s failure to comply will result in Lessor assessing a charge to
Lessee of $100.00 per dish removed; which amount shall be construed as liquidated damages
by the parties.
Compliance with the above terms and conditions are accepted by the authorized individual
representing Cable Company _________________________________________by affixing
their signature hereto.

__________________________________________________hereby authorizes Cable Company
__________________________________________________ to effectuate the aforementioned
installation according to the terms and conditions specified herein by affixing his signature
hereto.
